If you're just getting started, an elaborate, one sight-per-page
form like the ones in Mixter's _Primer of Navigation_ can be helpful.
This book has specific, take-you-by-the-hand forms for Noon, Polaris,
    and general sight reduction.
With more experience, you'll either eschew forms altogether, or go
to a more streamlined columnar form. For the latter, Davis publishes
good ones, printed on paper that will withstand a soaking (very
important). They have specific forms for HO-211, -229, and -249,
plus a "universal" form. The Celestaire catalog sells these -- see

Data General Corp., Westboro, Massachusetts (for whom I do not speak)
