But is that really an incident of GPS failure? A poorly installed navigation system was physically broken by a clumsy crew. And then a poorly trained bridge crew failed to recognize the inadequate warning message (icon change) on their plotting screen. That's not a GPS failure, that's an engineering and installation failure, compounded by a training failure. The failure is unrelated to GPS, per se. "A poor workman blames the tools".
