    Sorry, Hew. I quite misconstrued what you were asking for. You were
    absolutely right, those two useful reference pages were missing. The source
    from which I downloaded must have been corrupt in that way, and I just
    hadn't realised. So I'm unable to improve on what went out.
    
    Fortunately, Douglas has supplied a scan from a paper-copy, which includes
    those pages. I included the other images because the diagrams on his scan
    were somewhat unclear, but the text was readable. Between the two, then, you
    should be able to assemble all that's needed. Wonderful, what a bit of
    collaboration can do.
