This rings a bell. Checking my copy of AstroNavPC, from 2001, I find that I had penciled "A = C and B = 0" into the margin of p. 68. Is that it?
But why do you think "the whole concept of deducing an error-ellipse from a single cocked-hat is fundamentally flawed" ? The ellipse serves to measure the quality of the data set which is at the basis of that single cocked hat. Specifically, it does a better job of it than the cocked-hat itself.
