The blank space on your updated work form is about right for figuring LHA and declination from LTA data. The most compact LTA to laminate would be from Pub 249. There is a problem in that 2016 is the last year for accurate Sun data. If this LTA gets updated then it would be my first choice for use with the compact haversine table and azimuth graph.
