The first H.O. Pub. 9 Volume 2 came out in 1975 -- green covers and all.
It's compainion Volume 1 came out in 1977. Then, as you say, the next
revision was V2/1981 and V1/1984. I never compared the tables of contents
of the 1975 and 1981 V2's, but the 1981 V2 was considerably heftier (though
no better bound, but that's another gripe).
